You may use this form to report how much beer you’ve received from out-of-state breweries in the last six months.
This report is due twice a year:
- For purchases made between January 1 and June 30 of this year, the due date is July 15.
- For purchases made between July 1 and December 31 of this year, the due date is January 15 of the following year.
All records, inventories, invoices, sales records and delivery records must be kept for inspection by the Department of Revenue.
